Save time at check-in by completing a waiver online! We require a new waiver each calendar year, so if you haven't completed one yet in 2026, you can do so now and skip the paper waiver at check-in.

If you need to complete a waiver for a minor child participating in one of our after school or school break programs, please use the Minor Waiver instead.

WAIVER OF LIABILITY AND ASSUMPTION OF RISK

PLEASE READ CAREFULLY

I understand that my and my family’s participation in any Live Love event during 2026 is a voluntary activity, and that I am participating by my own free choice. In consideration of being allowed to participate in these activities, I hereby agree to assume the risks of property damage, injury, illness or death in any way associated with my participation in this activity. I agree to release, defend, indemnify, and hold harmless the Live Love Organization, its officials, employees, representatives, volunteers, and agents for any and all rights and claims for damages, including attorney fees, I now, or may hereafter have, and arising from or in any way connected with my participation in the Organization’s activities and programs.

In addition, I understand that for Live Love programs that are field trips, Live Love will transport my child to and from the location.

PHOTO RELEASE

Furthermore, I give my permission to have photos and/or video recordings taken of me or my child(ren) for publicity purposes during Live Love Organization’s activities even though we will not receive compensation of any kind for appearing in such photos or video recordings.

CAUTION

I acknowledge that I have carefully read this WAIVER OF LIABILITY AND ASSUMPTION OF RISK and fully understand that I am waiving any right that I may now or hereafter have to bring a legal action to assert any claim against the Live Love Organization in connection with my or my child’s participation in these voluntary activities and programs.
